Job Summary
The Veteran Community Liaison will serve as a vital resource for veteran patients and their families, ensuring they are aware of and can access the full range of veteran benefits, services, and resources available to them, particularly those related to the end-of-life process. The Veteran Community Liaison will build a strong community of veterans, working directly with community organizations and resources; while also working directly with veterans and their families to help navigate VA benefits, survivor support, burial benefits, and other related services, offering guidance, emotional support, and practical assistance.

Job Duties and Responsibilities
- Specific to Position:
- Has a solid understanding of basic hospice regulations, levels of care, and admission criteria.
- Utilize community resources to research and identify referral sources, potential hospice physician relationships, and missed marketing opportunities in the community.
- Has the ability to create new relationships by constantly seeking out new referral relationships and opportunities in the communities served.
- Gather data on patient referrals to assist with the admission process.
- Ability to make “cold calls” in person and by telephone.
- Excellent listening skills and ability to understand and empathize with patients and their families and the professional referral community.
- Works with the Admissions Team to ensure that patient admissions are completed in a timely and accurate manner. This could include attending the admission, obtaining consents, obtaining attending physician certification of terminal illness and other admission duties to be assigned, when necessary.
- Coordinates effective community education programs to increase community awareness on the benefits of hospice and palliative care and to help generate qualified patient referrals.
- Acts as a liaison between physicians, referral sources, and other referral partners to report patient’s progress towards goals, current condition, and updates to care plans when appropriate.
- Provide information and assistance to patients and families regarding VA benefits, healthcare options, financial resources, and posthumous benefits available to veterans and their families.
- Educate veterans and their families about the benefits they may be entitled to after the veteran’s passing, including survivor benefits, funeral and burial assistance, and memorial services.
- Act as the point of contact between hospice care providers and the Department of Veterans Affairs (VA), the Veteran Community Liaison will coordinate services and advocate for the needs of veteran patients and their families.
- Offer emotional and practical support to the families of veteran patients, assisting them in understanding their rights and accessing resources during a difficult time.
- Build relationships with local veteran organizations, VA representatives, and community partners to create a network of support for veterans and their families.
- Maintain accurate records of veteran resources provided, including benefits claims and follow-up assistance, and ensure proper documentation for compliance with hospice and VA guidelines.
- Stay up-to-date with the latest veteran resources, benefits, and regulations. Provide training and guidance to hospice staff on veteran-specific care needs and available resources.
- Educate hospice care providers on the end-of-life process for veterans and their families, also relating to diagnosis and treatment.
- Other duties as assigned.
Physical/Safety Requirements:
1. Participates in emergency procedures, including disaster drills.
2. Operation of equipment in a safe manner and shares responsibility for communicating safety violations or unsafe practices to management.
3. Equipment Used: General office equipment (telephone, copy machine, computer, fax machine) and motor vehicles.
4. Category 2 for potential exposure to blood/bodily fluids.

12. Utilize appropriate basic office equipment.
Requirements
Qualifications and Experience
- To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required to perform this job.
- Minimum Education: High School Diploma, Bachelor’s degree in Marketing, Public Relations or healthcare-related field preferred. Experience in lieu of education may be considered.
- Minimum of two years of experience in marketing, sales, public relations, or in a healthcare setting or as a veteran resource. Prior military experience required or strong evidence of knowledge relating to veterans and/or the military.
- Oregon State driver’s license required
- Auto Insurance required
- Proficiency in Microsoft Office Suite
