Job purpose
The NKC case manager will facilitate the client’s wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Based on the client's needs and values, the case manager will link clients with appropriate providers and resources throughout the community and empower them to achieve their personal goals.
Duties and responsibilities
· Conduct comprehensive assessments of clients to determine service eligibility and needs.
· Complete pantry check-ins and new client intake, update appointments and pantry referrals.
· Provide clients with appropriate referrals to NKC educational programs and/or other community resources.
· Utilize Empath Mobility Mentoring coaching tools when providing case management to clients.
· Attend all training necessary for the continuity of client services: MDHHS Navigator training, MEAP, Casebook, and others as required.
· Facilitate rental assistance, utility assistance, and all other services provided by NKC.
· Maintain accurate and up-to-date client records, maintain client database, and prepare monthly reports on client services as requested.
· Document case notes, updates, and outcomes in a timely and professional manner.
· Serve on committees relating to NKC mission and promote NKC in the community.
· Maintain regular contact with community service providers and stay abreast of new community resources.
· Attend meetings and trainings as required.
· Other duties as assigned by the Lead Case Manager.
Requirements
- Bachelor’s degree or combination 5 years schooling and experience in Social Work, Sociology, Psychology, or similar human services field required.
- Fluency in Spanish is strongly preferred.
- Minimum of one year of case management or comparable experience.
- Self-starter with a high degree of initiative.
- Excellent administrative and organizational skills; attention to detail and deadlines.
- Ability to form and establish positive partnerships in the community.
- Must work well in a team environment.
- Have a good understanding of issues related to individuals and families that face socioeconomic challenges.
- Must have an ongoing commitment to cultural competency.
- Creative and forward-thinking a plus.
