Discharge Planner Sign On Bonus Offered

• Job Overview:
• To provide quality case management and coordinate care for discharging patients.  This includes working directly with outpatient providers, programs, and agencies to assist patients in the development of an appropriate discharge plan. This position also involves interdisciplinary meetings, to work as a team for the best patient outcome.

Requirements

• Minimum Education High School diploma required; Bachelor's preferred
• Minimum Work Experience 2 years experience in a health care setting preferred, but not required.
• Licenses/Other Requirements Must be at least 21 years of age, and able to complete all pre-employment testing.
• Safety Sensitive: This is a safety sensitive position.  A safety sensitive position refers to any job that is directly related to patient care or jobs that WBHH believes in good faith could affect the safety or health of the employee performing the task, or others.
• Full time, 40 hour position with sign-on bonus offered
