Responsibilities

• Delivers high-quality therapy services in accordance with the relevant Standards of Practice.
• Conducts patient evaluations and formulates individualized treatment plans, encompassing both short-term and long-term objectives.
• Monitors and communicates the patient's progress, responses to treatments, and any changes in their condition to the responsible physician; adjusts the treatment plan as necessary.
• Provides guidance to the patient, caregiver, and other healthcare team members on strategies to enhance and correct disabilities.
• Maintains regular communication with physicians and the broader healthcare team.
• Facilitates patient discharge planning by establishing realistic and achievable long-term goals that can be adjusted as the patient's condition evolves.
• Prepares the patient and their caregiver for post-discharge care.
• Reports any significant concerns related to patients, families, or physicians to the Clinical Director.

Requirements

• Must hold current state licensure, CPR certification, driver’s license, and automobile liability insurance.
• Thorough knowledge of professional theory, practice, techniques, principles, and equipment.
• Effective oral and written communication skills. Ability to document patient care effectively.
• Ability to establish and maintain a working relationship with patients, other health care professionals, physicians, and the public.
• New Grads considered, 1+ year of experience preferred
