LVN - Home Care Coordinator

Key Responsibilities:

- Coordinate Home Care Services for participants using home care agencies

- Conduct nursing care within LVN Scope of practice

- Provide general nursing care in the clinic and participant’s home

- Document in the EMR within set documentation timeline

- Communicate with home care employees to understand participant’s condition

- Collaborate with the Transportation Coordinator for participant transportation

- Schedule Home Care appointments and support scheduler for other appointment needs

- Oversee visits completed by external agencies

Schedule

- Monday to Friday, 8a-5p

Requirements

- Associate degree or higher

- Valid Licensed Vocational Nurse (LVN) in the state of California (CA)

- 3 years of experience in Home Health Care or Home Health

- 2 years of experience in Geriatric Care, Senior Care, or Geriatric/LTC

- Valid American Heart Association Basic Life Support (AHA BLS) or American Red Cross Basic Life Support (ARC BLS)

- Proof of a valid CA driver’s license, personal transportation, good driving record, and auto insurance as required by State law
