Senior Helpers of Coastal Orange County is seeking a dedicated Care Manager / Care Giver / Private Home Care to provide exceptional support to our clients in their homes. In this multifaceted role, you will assist with daily activities, create personalized care plans, and offer companionship to ensure a high quality of life for our clients. You will be responsible for fostering a nurturing environment while working closely with families and healthcare professionals.
Responsibilities
- Provide personal care services including bathing, grooming, and meal preparation
- Implement and monitor care plans tailored to the individual needs of each client
- Assist clients with mobility and ensure safety within their home
- Offer companionship and engage clients in meaningful activities
- Communicate regularly with healthcare professionals and family members regarding client progress
- Participate in care meetings and training sessions as required
- Maintain accurate records of care provided and any changes in client conditions
Requirements
- Previous experience as a caregiver or in a related field
- Strong understanding of medical terminology and health care practices
- Excellent communication and interpersonal skills
- Compassionate and patient demeanor
- Ability to follow care plans and document care activities accurately
- Valid driver's license and reliable transportation
- Relevant certifications (CNA, HHA, etc.) preferred
- Completed a Background screening with Guardian Dept. of Social Services, or be able to pass a Background screen with the Dept. of Social Services.
