About the Role:
As a Registered Nurse for the Program of All-inclusive Care for the Elderly (PACE), you will play a crucial role in managing and delivering direct nursing care to PACE participants in various settings such as clinics, care centers, homes, virtually, SNFs, and beyond. You will work closely with the Interdisciplinary Team (IDT) to conduct assessments and develop personalized care plans that prioritize quality, outcomes, teamwork, and empathy.
