Responsibilities
- Delivers high-quality therapy services in accordance with the relevant Standards of Practice.
- Conducts patient evaluations and formulates individualized treatment plans, encompassing both short-term and long-term objectives.
- Monitors and communicates the patient's progress, responses to treatments, and any changes in their condition to the responsible physician; adjusts the treatment plan as necessary.
- Provides guidance to the patient, caregiver, and other healthcare team members on strategies to enhance and correct disabilities.
- Maintains regular communication with physicians and the broader healthcare team.
- Facilitates patient discharge planning by establishing realistic and achievable long-term goals that can be adjusted as the patient's condition evolves.
